REGISTRATION REQUIREMENTS

WHO MAY REGISTER THE STUDENT?

The student’s parent, legal guardian, or legal custodian should enroll the student.  The parent, guardian, or legal custodian enrolling the student under the provisions of O.C.G.A. 20-2-690.1 (or the student in the case of an emancipated minor) must be a full-time resident of Schley County.

WHAT DOCUMENTS ARE NEEDED TO ENROLL A CHILD IN SCHLEY COUNTY SCHOOLS?

**Please have the following documents in hand when you come to register your child for school. **

1. PROOF OF RESIDENCY:

If you own your residence please provide the following three documents. 

1. Two of the most recent utility bills (gas, electric, water, or cable are acceptable utilities) in your name, and 

2. A deed, property tax statement, mortgage statement, or settlement statement in your name. 

If you lease or rent your residence please provide the following three documents. 

1. Two of the most recent utility bills (gas, electric, water, or cable are acceptable utilities) in your name, and

2. A current lease/rental agreement signed by the owner of the residence. 

If you are unable to provide both proofs of residency in your name, please complete a Schley County Schools Affidavit of Residency. (Notaries are available at the Board of Education) 

The following three documents must be provided in addition to the Affidavit of Residency.

1. Two of the most recent utility bills (gas, electric, water, or cable are acceptable utilities) in your name, and

2. A deed, property tax statement, mortgage statement, or settlement statement of the owner of the residence.

2. PROOF OF STUDENT'S AGE:

The school system requires evidence of the student’s date of birth and accepts as evidence a certified original birth certificate, a certified hospital issued birth record or an alternate document from the prioritized list in the state enrollment rule. Birth certificates for children born in Georgia may be requested from the Schley County Probate Court (229.937.2905). Birth certificates for children born in other state may be obtained by contacting the Vital Records Department within that state. 

3.  IMMUNIZATION CERTIFICATE:

A current and completed REVISED Georgia Department of Human Resources Form 3231 is required by state law showing immunizations for measles, mumps, rubella, polio, whooping cough, tetanus, diphtheria, hepatitis B, and varicella. This form may be obtained by taking the current immunization record to the Schley County Health Department located at 45 Oglethorpe Street, Ellaville, GA 31806 (229.937.2208) or a Georgia physician.

4.  CERTIFICATE OF HEARING, VISION, DENTAL EXAMINATION:

A current (within 1 year of school entry) and complete (hearing, vision,and dental exam) Georgia Department of Human Resources Form 3300 is required by state law. This form is available from the Schley County Health Department located at 45 Oglethorpe Street, Ellaville, GA 31806 (229.937.2208) or a Georgia physician.

5.  COPY OF SOCIAL SECURITY CARD:

A copy of the child's Social Security card, the Social Security number, or a completed waiver form, must be presented for enrollment.  To obtain a lost card, please call the Social Security Administration (800.782.0059) prior to enrolling your child at Schley County.
